Advanced Directive - Florida -(Living Will and Healthcare Proxy)
 
An Advanced Directive in the State of Florida consists of two healthcare-related, interactive, written and witnessed legal documents. When utilized together they ensure your end of life care treatment is followed to your exact wishes by a person which you choose. 

 
A1. Living Will - This document is utilized to clearly and specifically detail the type of healthcare that you want (or do not want) if you should develop a terminal condition or become incapacitated and can no longer make your own medical decisions.
 
A2. Healthcare Proxy- Within this document you select a Healthcare Surrogate/Agent who becomes responsible for your medical care including life support and other personal healthcare wishes detailed within your Living Will when you are deemed incapacitated and are no longer able to make these decisions for yourself.
 
Also known as a Medical Durable Power of Attorney it provides your physicians the authority to discuss with your chosen agent information regarding your medical condition, second opinions, medical options and future opportunities available for you.
 
Once you have executed both your Living Will and your Medical Durable Power of Attorney you have both successfully detailed your healthcare wishes and provided a trusted person that you have chosen the ability and power to play a vital role in your medical care.

B. Last Will and Testament - A legal document executed to specifically state your final wishes regarding your body and disposition of your assets and property.  Within this document you appoint an Executor to carry out your personal wishes and name Beneficiaries whom you have chosen to receive your property and assets.
 
Once you have executed your will you can rest with confidence knowing your exact wishes have been detailed, your loved ones cared for and all of your financial matters will be handled as you have set forth.

C. Financial Durable Power of Attorney - A document where you, the Principal authorize a trusted person with the legal authority to handle your finances and property in case you are unavailable or should become incapacitated.  Within this document you detail his/her exact authority and can even list the responsibilities your Financial Durable Power of Attorney will have.  It is important to make this document durable so that it does remain in effect should a doctor decide you lack the ability to make your own decisions or become incapacitated. 
 
By executing a Financial Durable Power of Attorney you provide yourself with a private, simple, inexpensive and reliable way to arrange for someone you trust to manage your finances if you are unable to do so.
