A1. Living
Will - This document is utilized
to clearly and specifically detail the type of healthcare that you want (or do not want) if you should develop a terminal
condition or become incapacitated and can no longer make your own medical decisions. A2. Healthcare Proxy- Within this document you select a Healthcare Surrogate/Agent who becomes responsible for your medical care including
life support and other personal healthcare wishes detailed within your Living Will when you are deemed incapacitated and are
no longer able to make these decisions for yourself. Also known as a Medical Durable Power of Attorney it provides your physicians the authority
to discuss with your chosen agent information regarding your medical condition, second opinions, medical options and future
opportunities available for you. Once you have executed both your Living Will and your Medical Durable Power of Attorney you have both
successfully detailed your healthcare wishes and provided a trusted person that you have chosen the ability and power to play
